Add custom intake form fields.

Steps

Section titled “Steps”
  1. Go to Personal Link settings
  2. Click “Add Field”
  3. Configure the field:
    • Label (question text)
    • Type (text, dropdown, etc.)
    • Required or optional
    • Placeholder text
  4. Save

Best Practices

Keep forms:

  • Short (3-5 fields max)
  • Relevant to conversation
  • Clear and simple
